*It is very important to us to provide you with the best  service possible. In order to assure quality service, we ask that all of your submissions include the following: 

• Complete accord application including the excess liability section
  • 5 years currently valued loss information
  • Detailed description of operations for each named insured
  • Number of vehicles: type, radius, use, year, vin # and gross vehicle weight
• Underlying insurance carriers, premiums, limits, terms and conditions
